Right to Amend
You may request that we amend your PHI if you believe it is incorrect or incomplete. We may deny your request in certain circumstances.
Right to an Accounting of Disclosures
You have the right to request a list of certain disclosures we have made of your PHI for the six years prior to the date of your request.
Right to Request Restrictions
You may request restrictions on certain uses and disclosures of your PHI. We are not required to agree to requested restrictions, except in limited circumstances required by law.
Right to Confidential Communications
You may request that we communicate with you about your health information in a certain way or at a certain location.
Right to a Paper Copy of This Notice
You have the right to a paper copy of this Notice at any time, even if you have agreed to receive it electronically.

We implement robust safeguards to protect your PHI, including:

5. Breach Notification

7. Complaints
If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services Office for Civil Rights. You will not be penalized for filing a complaint.
